The present study is an applied research aimed at examining the effectiveness of the Intensive Short-Term Dynamic Psychotherapy (ISTDP) approach on post-traumatic stress disorder (PTSD) in individuals with dissociative disorders. This was conducted by administering propranolol or a placebo once a week for six consecutive weeks to reduce PTSD symptoms among individuals with PTSD with or without high dissociative symptoms. To achieve this, a randomized clinical trial was conducted on 66 adults with a long-term diagnosis of PTSD, utilizing the SCID PTSD module, the PTSD Checklist (PCL-S), the Beck Depression Inventory II, and the Dissociative Experiences Scale. Patients with and without high dissociative experiences showed significant improvement in their PCL-S scores during the six therapy sessions, and PCL-S scores decreased across all patients during the post-treatment period. However, no correlation was found between the presence or absence of high dissociative experiences and any specific effect of propranolol treatment. Interestingly, patients who had high dissociative experiences prior to treatment demonstrated a very significant improvement in their Dissociative Experiences Scale (DES) scores after six therapy sessions, and these patients maintained this improvement three months post-treatment. The intensive short-term psychodynamic approach is an effective method for treating dissociative symptoms in patients with PTSD, and the improvement of these dissociative symptoms was associated with a reduction in the severity of PTSD.
